Slate Form

UC Slate Embed allows you to embed a Slate form onto your web page.

This plugin is enabled upon request. Please email webdev@uconn.edu with your website URL to request this plugin. Once enabled, you can add the following short code on your page to embed a Slate form.

[slate host="your Slate form host" id="your Slate form ID#"]

Using https://connect.abcd.uconn.edu/register/?id=sk235bb-arv43-6n7-h5h4h as an example of a slate form URL, you would derive the following short code:
