Creating Forms in Ultra Course View

This article is for instructors looking to create Forms in their Blackboard Ultra course. Forms can be utilized to gather input and feedback from students.

Overview

  • The Form tool is similar to the Survey tool in Blackboard Original courses.

    • In Ultra, instructors can choose to make the Forms submissions anonymous or not.

  • A Form will also have a corresponding Gradebook column which, by default, is ungraded (no point value) and set as Complete/Incomplete grade display.

    • In Form settings, it can be made into a gradable item by clicking the checkbox for Grade form.


Creating a Form

  1. Navigate to your HuskyCT Ultra course.

  2. Hover your cursor over the location where you want to create a Form. Click the plus sign to the left of the line.

  3. On the dropdown that appears, click Create.

  4. Under Assessment, click Form.

  5. By default, the form will be named according to its creation date. To change the name of the assignment, click the default name.

     

  6. To add questions to the form, click the plus sign at the center of the screen.

     

  7. From the dropdown options, select a question type. *See the next section for more detail on Question Types.

  8. Click the gear icon to adjust the Form Settings including due date, Grading, and anonymous submission options.

     

Question Types:

A Essay questions is a question where the response is a sentence or paragraph.

 

  1. Type a question in the textbox.

  2. Save.

You can use Likert questions on forms to measure students' attitudes or reactions with a comparable scale.

  1. Type a statement or a question in the textbox.

  2. Choose Scale Range from 3 to 7.

  3. Clarify options.

  4. To include the Not applicable option, click the checkbox.

  5. Save.

The question will be shown to students like the image below:



Multiple Choice questions require students to choose one or more answers from a list of provided answers.

  1. Type a question in the textbox.

  2. Type the options. The default number of options is three. If you would like to create options more than three, click Add Option.

  3. You can also reorganize the order of the options by dragging the up and down arrows.

  4. Save.

True/False questions prompt students to response with either True or False to a given statement.

  1. Type a statement in the textbox.

  2. Save.

 

 

Viewing Form Submissions

Form responses can be viewed by student, by question, or as an aggregated spreadsheet file.

  • For anonymous Forms, instructors can view individual student responses, with names removed.

  • For non-anonymous Forms, instructors can view an individual student’s responses.

  • In all cases instructors can download the aggregated responses.

Viewing Form Activity in the Gradebook

  • Prior to a set due date, the Gradebook column for anonymous forms will display “Anonymous” for all students, whether they have submitted yet or not.

  • After the due date has passed, the Gradebook column will display a Complete/Incomplete status. Students who have submitted will have a green check mark. Those who have not will display as a ( - ).
