To order a replacement for your device, you can select the Replacement Orders or Review and/or Modify Existing Lines of Service link on the landing page. Both will take you to the same place.



You will be brought to the Service Request Tool as listed above. Enter the user's name or number you wish to order a replacement for. Select Lookup Line Information to populate the user's information.

If you scroll to the bottom of the page, you can select Order Full Cost Replacement and click on Save and Continue.
The page will expand to list out all available devices for you.

Check off that you have read the disclaimer, and click the Select Device button to choose a device.

Once you have selected a device, you will see an option below to select additional accessories. Once checked off, this should automatically enlarge the form to see available accessories below.

Select any accessories you may need.After selecting your device and accessories, the form below will auto-fill with the user's information. Please complete the fields as necessary. If you need to make any changes to the name/address/etc., you can edit that here.

Select Save and Continue to move on to the next step.


You will be brought to the final review for your request. You can select Submit Requests to send off your request for a replacement device to the team at Motus. Once we receive your request, we will work with you to process the request and deliver your replacement device.
