Understanding the Ticket Form

The student that is having the issue should fill out the form.

  • Name: The name of the student.
  • Email: Please use your UConn email if possible.
  • Version: See "i" or "About" button in LockDown Browser.
  • Institution Name: University of Connecticut
  • LMS Web URL: lms.uconn.edu
